A Reminder. It's likely that there exist equivalent English terms that are officially in use by the Malaysian Department of Registration. Since I'm just doing a public (and free) service by preparing a rough translation so don't quote me (I'm not even married so I don't know what marriage certificate is all about) since I might be wrong especially if you're planning to prepare a legally binding document or something.
1. Full name of the Groom
2. Full name of the Representative of the Groom
3. Full name of the Bride
4. Previously unmarried or previously married
5. If previously married please provide statement
6. Full name of the wali/representative (Is there an english term for wali ? basically it means the one that give away the bride/groom i.e father, grandfather)
7. Relationship (obviously referring to the matter above)
8. Full name of the witness
9. The name of the place where the marriage took place
10. The name of the people who married the couple (the kadi, I'm not sure about this translation)
11. The amount of mahar (obviously a muslim marriage, the mahar or dowry paid by the Groom is based on the rates set by each states in Malaysia)
12. Cash/delayed payment (for the mahar which is really weird since it is such a token sum if compared to the amount spend on the narriage recption itself)
13. Purchases and price
14. Total wedding cost
15. Ta`lik if there is any
16. Date of marriage and time
17. Signature of the Registration Officer
18. Seal of the Registration Officer
19. Testimonial letter
26. Signature of the Yang di pertua High Court of Justice OR commisioner of oath.
27. The Seberang Perai Muslim Marriage/Divorce Registration Officer (Seberang Perai is a territory in the state of Penang)
28. Statement for the record
29. Other statement if any
